CMS Demonstration: Community-Based Care Transition Program

The Affordable Care Act (ACA) of 2010 has prompted a national focus on 30 Day Hospital Readmissions and Patient Care Transitions. Decreasing payment to hospitals with excessive rates of 30 Day Readmissions for Acute Myocardial Infarction (AMI), Heart Failure (HF) and Pneumonia is one cost containment measure in the ACA.

Another ACA measure (section 3026) is aimed at offering financial assistance to hospitals in the 4th Quartile for 30 Day Hospital Readmissions. Known as the Community Based Care Transition Program (CCTP), this program is to:

  • “Reduce hospital readmissions,
  • Test sustainable funding streams for care transition services,
  • Maintain or improve quality of care; and
  • Document measurable savings to the Medicare program.”

Hospitals that are eligible to participate in this Demonstration will be required to partner with Community-based Organizations (CBO). During a December 3, 2010 CMS National Conference on Care Transitions, CBOs were defined as “organizations that provide care transition services across the continuum of care through arrangements with subsection (d) hospitals.”

National & MMP Client data was taken from Hospital Compare's current reporting time frame of 7/1/06 - 6/30/09.

Collectively, MMP, Inc. Clients have higher than national or state 30 Day Readmission Rates for AMI and HF and lower Readmission Rates than national and state for Pneumonia.

The two hospitals highlighted in red are eligible to participate in the CCTP Demonstration.  Their 30 Day Readmission rate was above the national and state average for all three diagnoses.  It is also important to note that several of the remaining hospitals have higher than national and state 30 Day Readmission rates for one or two of the diagnoses. 

This Demonstration was set to begin January 2011.  At the time of this email blast the application forms have not been made available by CMS.   

 

What should hospitals be doing now?  

 

·         Be knowledgeable about your hospital’s 30 Day Readmission Rates for AMI, HF and Pneumonia.  These rates have been publically reported on the Hospital Compare web site since July 2009. 

·         Go to the Community Based Care Transition Program Medicare Demonstration site at: http://www.cms.gov/demoprojectsevalrpts/md/itemdetail.asp?itemid=cms1239313 where you can:

o   Access the December 3, 2010 Care Transition Conference Presentations

and  

o    Determine if you are in the 4th Quartile of Hospitals in your state for 30 Day Hospital Readmissions.

·         Take action now to reduce medically unnecessary hospital readmissions.
