The drugs listed below are being added to the Cahaba GBA ‘Self-Administered Drug (SAD) List’.

Fuzeon® Injection, enfuvirtide, 1 mg J1324 Rationale: SC injection administered by patient twice daily for a chronic condition.
Increlex®, Iplex™ Injection, mecasermin, 1 mg J2170 Rationale: SC injection administered by patient daily/twice daily for a long-term condition.
Muse® Alprostadil urethral suppository J0275 Rationale: Apparent on its face. Urethral suppository administered by patient on an as needed basis.
Somavert® Pegvisomant for injection Use J3490 Rationale: SC injection administered by patient daily for a chronic condition.
